The order of 31 July 2025 publishes the CERFA forms and provides various other information relating to the files to be transmitted.

Request for opinion file

For the file requesting an opinion from the authority organizing the reception of the young child, the CERFA form n°17579*01 and the supporting documents it mentions are to be transmitted to the municipality of implantation.

The decree adds that in the event of a favorable opinion, the organizing authority shall indicate:

• the name of the applicant;
• whether the establishment or service is managed in the context of a public service delegation or a public contract (and, where appropriate, the identification of the contracting public authority);
• the address or proposed location of the establishment or service;
• the type of establishment or service;
• the accommodation capacity and the category of establishment or proposed service;
• the characteristics of the reception project, the age limits of the children that can be accommodated and the days and hours of operation (in terms of the method of charging families).

Application file for authorization to establish an institution or a service for the care of young children

With regard to the application for authorization to create, extend, transform, renew the authorization, modify or change the manager of the establishment or early childhood care service, the order states that the CERFA form n°17580*01 and provide supporting documents. The file must be sent to the president of the departmental council of the department in which the establishment or the service for young children is located.
