To be used first by the developer (or project manager), who must fill in the left-hand part (DT), and then by the works performer (who may be a company or an individual), who must complete the right-hand part (DICT).
Before carrying out work in the vicinity of one or more networks or pipelines, it is necessary to send a prior declaration to the operators concerned. This is done after consulting the network census online service or a provider contracted by the one-stop shop.
Replaces Cerfa No. 13619*01 (former 90-0189) and Cerfa No. 13618*01 (former 90-0188), which are no longer in force.
