This form allows any establishment producing or marketing food containing products of animal origin to make the mandatory declaration before opening.

It is also mandatory for each change of operator, address or activity.

The form must be downloaded, printed and completed.

The request must be addressed to the Departmental Directorate for the Protection of Populations (DDPP).
