The flow manager of the attachment tobacco who no longer wishes to supply manufactured tobacco products to a reseller establishment must complete this form.

Document to be completed and signed in 1 original copy.

This copy is to be given to the retailer who keeps it.

The reseller must send a copy to the local customs and indirect duty department to which he is attached.
