Certificate on the honor of the spouse (married, former or common-law partner) of the head of company who is a farmer (choice of status) (Model document)

This form is for the spouse of thefarmer.

Once completed, the certificate must be signed (by hand) by the spouse (married), civil partnership partner or common-law partner).

The certificate shall be sent directly by the one-stop shop or by CFE: titleContent to your social security organization.
