Subscribe to the College of Physicians Table (Form)

Registration on the list of the Ordre des médecins is mandatory to practice medicine in France.

The file must be sent by registered letter with request of receipt or deposited with the departmental council of the Medical Association of the place of practice.

The registration form must be printed and completed by hand. It must be accompanied by the following documents:

• Photocopy of a identity document valid
• Copying securities and certificates accompanied, where appropriate, by a translation into French, made by a translator authorized by the French courts or authorized to intervene before the judicial or administrative authorities of an EU or EEA Member State or Switzerland
• Extract from criminal record and certificate of good professional status, less than three months old and issued by a competent authority of the State of origin or of provenance
• Declaration on Honor certifying that no proceedings which may result in a conviction or sanction which may have an effect on the inclusion in the schedule are pending against you
• Certificate of Cancelation, Registration or Registration issued by the authority with which you were or are registered or registered less than 3 months old OR one declaration of honor certifying that you have never been registered or registered, OR, failing that, a certificate of registration or registration in an EU or EEA State less than 3 months old
• All elements that establish that the level of linguistic knowledge sufficient for the exercise of the profession
• Updated and detailed curriculum vitae.

You then have to make an appointment with the departmental council to finalize the registration procedure
