Application for authorization to use the ATFMX endorsement in a flight plan (Form 15219*01)

For operators wishing to use the descriptor ‘ATFMX’ after the indicator ‘STS/’ in field 18 of the flight plan (see AIC France A 14/14). The information required in this form and any supporting documents must be sent to the email address atfmx@aviation-civile.gouv.fr at least 48 hours (working period) before the flight departure time *; in case of emergency, as soon as possible.

This form applies to flights operators that intend to use the ‘ATFMX’ descriptor after the ‘STS/’ indicator in field 18 of the flight plan (see AIC France A 14/14). The application and when possible supporting documentation must be sent to the e-mail address atfmx@aviation-civile.gouv.fr at least 48h (working period) before the flight departure*; in case of urgency, as soon as possible.
