Funeral Operations - Prior Declaration (Form 16048*01)

Allows a relative of the deceased in charge of organizing the funeral or a funeral operator to make prior declarations in the town hall.

The relative of the deceased or the funeral operator must provide a set of supporting documents.

The declaration, completed online and accompanied by supporting documents, can be filed on site or sent by mail or email to the competent city hall.
