The file should include the following documents:

• Completed and signed prior service declaration form
• Photocopy of a valid ID
• Photocopy of the original of the doctor’s diploma, accompanied by a translation into French by a sworn translator
• Photocopy of the original of the title of specialist doctor, accompanied by a translation into French by a sworn translator
• Where the evidence of formal qualifications has been issued by a third State and recognized in a Member State of the European Union or a party to the Agreement on the European Economic Area, other than France: recognition of the evidence of formal qualifications of a doctor and of a specialist doctor, all the relevant documents proving that he has pursued the profession in that State for three years full-time or part-time for an equivalent total period
• where possible, a certificate from the competent authority of the Member State of establishment of the European Union or party to the Agreement on the European Economic Area or Switzerland certifying that the diplomas possessed by the doctor attest to training in accordance with Articles 24 and 25 or 28 of Directive 2005/36/EC as amended, accompanied by a translation into French by a sworn translator
• a certificate issued less than 3 months ago by the competent authority of the State of establishment of the Member State of the European Union or party to the Agreement on the European Economic Area or of Switzerland certifying that the doctor is legally established in that State and that he is not liable, when the certificate is issued, to any prohibition, even temporary, from practicing, accompanied by a translation into French made by a sworn translator
• Curriculum vitae
• Declaration on the honor certifying that the doctor has sufficient command of the French language for the practice of medicine in France and any evidence to establish that he possesses the linguistic knowledge necessary for the practice of the profession
• Certificate of insurance under professional civil liability covering the acts to be performed in France, under the same conditions and guarantees as those required in France or proof of exercise within a public health institution in France and, where appropriate, proof that the doctor will be covered by the insurance of that institution.

The file should include the following documents:

• Form for prior declaration of provision of services, duly completed and signed, accompanied, if possible, by a photocopy of the form sent during the previous application
• Photocopy of a valid ID
• Certificate from the competent authority of the State of establishment of the European Union or of the State party to the Agreement on the European Economic Area or of Switzerland, less than three months old, certifying that the doctor is legally established in that State and that he is not liable, when the certificate is issued, to any prohibition, even temporary, from practicing, accompanied by a translation into French made by a sworn translator
• Curriculum vitae
• Certificate of insurance in respect of professional civil liability covering the acts to be performed in France, under the same conditions and guarantees as those required in France or proof of exercise within a public health establishment in France and, where appropriate, proof that the doctor will be covered by the insurance of that establishment
