General case
The form is to be sent to the city hall, at least 15 days before the opening of the establishment.

In Paris
The form is to be sent to the police prefecture, at least 15 days before the establishment opens.
In Alsace-Moselle
The form must be sent to the prefecture at least 15 days before the opening of the establishment.
The prefecture is the one where the company is domiciled.
