Declaration of possession of birds in an outbreak of avian influenza (Form 15472*02)

Every holder of domestic birds (poultry or birds of pleasure) is required to make a declaration to the mayor of the place where the birds are kept.

Keepers of birds kept permanently in premises for home or office use shall not be required to make this declaration.
