This applies to establishments placing on the market products of animal origin or foodstuffs containing products of animal origin.

It is only the foodstuffs marketed to other professional establishments and not to the final consumer.

You must download the form, print it, fill it out and then send it to the Departmental Directorate of Population Protection (DDPP).
